This role could be full-time or part-time will include patient care rounding with paid medical directorship responsibilities in skilled nursing facilities in the Spokane, Washington, area. You will be a part of a physician/advanced practice clinician care team providing primary care in a post-acute/rehab setting. Care includes but is not limited to H&P, admissions, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer patients to the level and site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care. Our clinicians work closely with hospice, palliative care, and home care agencies to ensure a safe transition home. Our clinicians are also responsible for timely documentation and billing that meets Medicare guidelines through TeamHealth's EMR (gEHRimed). As a TeamHealth post-acute medical director, you will provide oversight, guidance, and mentorship to our clinicians within your facilities. In this role, you will conduct clinical case reviews as needed and share tools, information, and guidelines as they relate to quality of care, represent TeamHealth on appropriate external levels identifying, engaging and establishing/maintaining relationships within the healthcare community, and collaborate with clinicians, leadership and others regarding multidisciplinary care and services for patients.
