Job Description

Join our hybrid virtual care team on a PRN basis! Responsible for providing high-quality patient care to various patient populations within the framework of Acute Care Transitions' (ACT) vision, mission, and values.You will be providing protocol driven assessment, care coordination and interventions as appropriate informed by ACT Home protocols for diverse patient populations with a focus on oncology, medicine and post-operative care.You should be able to exhibit sound clinical decision making and critical thinking skills, maintain and practice in accordance with clinical guidelines per state and hospital regulations, and support research efforts to advance knowledge and promote evidence-based practice.You will be utilizing advanced nursing/medical knowledge to identify, prevent or solve complex and/or recurring patient care problems, writing orders which are in accord with current guidelines, and maintaining and practicing in accord with practitioner guidelines per state and hospital regulations.You will be expected to provide consultation to both nurses and physicians and serves as a resource to staff in area(s) of expertise, promote professional practice and a culture of safety; willingness to engage in process improvement efforts, obtain certifications as required by ACT Home and facilities, and perform other duties and responsibilities as assigned by the medical director or APC director.

    Hybrid Telehealth/Home-2-Health

    The nurse practitioner (NP)/physician assistant (PA) is responsible for providing high-quality patient care to various patient populations within the framework of Acute Care Transitions' (ACT) vision, mission, and values. The NP/PA serves as the point of contact representing the program to the patients and family in-person, collaborating with the attending physician in the organizations Command Center Mission Control and the patient's primary care physician (PCP). The NP/PA performs the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, the NP/PA will collaborate with the attending physician and the PCP to develop and implement an acute plan of care in the home.
