Join our hybrid virtual care team on a full-time basis! You will be responsible for providing high-quality patient care to various patient populations within the framework of Acute Care Transitions' (ACT) change to TeamHealth vision, mission, and values. You will be providing protocol driven assessment, care coordination and interventions as appropriate informed by ACT Home protocols for diverse patient populations with a focus on acute inpatient medicine. Clinicians perform the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, the nurse practitioner (NP) or physician assistant (PA) will collaborate with the attending physician and the PCP to develop and implement an acute plan of care in the home. You should be able to exhibit sound clinical decision making and critical thinking skills, maintain and practice in accordance with clinical guidelines per state and hospital regulations, and support research efforts to advance knowledge and promote evidence-based practice. Must be able to provide 2 years of current inpatient case logs. You will provide inpatient level acute care in the home setting in collaboration with the home hospital attending physician and RN care coordinator.
