Join our hybrid virtual care team on a part-time basis! You will be responsible for providing high-quality patient care to various patient populations within the framework of Acute Care Transitions' (ACT) vision, mission, and values. You will be providing protocol driven assessment, care coordination and interventions as appropriate informed by ACT Home protocols for diverse patient populations with a focus on oncology, medicine and post-operative care. Clinicians perform the initial face-to-face evaluation and assessment of potential patient admissions, and upon admission, you will collaborate with the nurse practitioner (NP) or physician assistant (PA) and the PCP to develop and implement an acute plan of care in the home. You should be able to exhibit sound clinical decision making and critical thinking skills, maintain and practice in accordance with clinical guidelines per state and hospital regulations, and support research efforts to advance knowledge and promote evidence-based practice. Facilitating the triaging of eligible, appropriate patients referred for a potential admission facilitated by ACT home guidelines and obtaining additional diagnostic and specialty consultation when needed are expected in this role. You will provide inpatient level acute care in the home setting in collaboration with the home hospital attending RN care coordinator. You will be utilizing advanced medical knowledge to identify, prevent or solve complex and/or recurring patient care problems, writing orders which are in accord with current guidelines, and maintaining and practicing in accord with practitioner guidelines per state and hospital regulations. You will be expected to provide consultation to both nurses and physicians and serves as a resource to staff in area(s) of expertise, promote professional practice and a culture of safety; willingness to engage in process improvement efforts, obtain certifications as required by ACT Home and facilities, and perform other duties and responsibilities as assigned by the medical director.
