Job Description

Our advanced practice clinicians (APC) are responsible for driving high quality, safe and cost-effective care of patients in our local post-acute care communities. As a nurse practitioner (NP) or physician assistant (PA), you will work on an interdisciplinary team rounding on all patients as medically necessary as required by CMS. This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer patients to the level & site of care most appropriate for their medical needs upon discharge including returning to home, independent living centers, assisted living, and/or memory care. Our APCs work closely with hospice, palliative care, and home care agencies to ensure a safe transition home. Our APCs are also responsible for timely documentation and billing through our EMR (Gehrimed) that meets Medicare guidelines. As an advanced practice clinician, you will be part of an integrated care team providing primary care services at post-acute care facilities including skilled nursing, assisted living and other long-term care facilities. Our patient population includes long-term care, short-term rehab, memory care, and clinically complex patients and residents. Being action oriented and having a "can do" attitude with a high standard of quality and safety in care is a top priority to help build a strong reputation in our community to support our patients.
