Work as part of our integrated care team to provide primary care services to patients who need long-term care, short-term rehab, memory care, and more. Responsibilities:- Drive high-quality, safe, and cost-effective patient care in local post-acute care communities
- Work in skilled nursing facilities, seeing 10-15 patients per day (Monday through Thursday, 4-day week)
- Collaborate within an interdisciplinary team, rounding on patients as medically necessary
- Handle documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
