Responsibilities:- Rounding between 2 facilities available 3 days a week on a part-time basis to start in area
- Patient volume 15-20 per physician per day
- Provide primary care services in post-acute care facilities, including skilled nursing and assisted living?????
- Collaborate with interdisciplinary teams and follow CMS guidelines
- Provide oversight, guidance, and mentorship to clinicians within facilities
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Drive high-quality, safe, and cost-effective patient care
Qualifications:- Background in providing primary care in internal medicine (IM), family medicine (FM), emergency medicine (EM), urgent care, physiatry, PM&R, or geriatrics.
- Ability to work flexible schedules
