We are seeking an NP or PA to work in a skilled nursing facility rounding 2-3 days/week. This opportunity includes the flexibility to set your own schedule. We have no start/stop times so you can round in the morning or in the afternoon and still have great work-life balance.
Key Responsibilities:- Perform primary care in a skilled nursing facility setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare providers
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes

Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated salary range of $45,000 to $65,000 annually with no cap on productivity income potential.
