Job Details:- Working in the town of Richmond
- 5-10 patients per day
- Full-time position
- Competitive compensation, with an estimated salary range of $78,000 annually to $84,000 annually with no cap on productivity income potential
Responsibilities:- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Qualifications: - Willing to train experienced candidates
- Two years of psyche experience and SNF experience ideal
