This part-time primary care role will include patient care rounding in a post-acute care setting in a skilled nursing facility (SNF) located in the Chesterton, Indiana area.
About the Role:As a physician in a skilled nursing facility environment, you will be an integral part of our care team, providing comprehensive primary care services to our residents. Your responsibilities will include:
- Conducting thorough patient assessments and developing individualized care plans
- Prescribing medications and treatments
- Collaborating with physicians and other healthcare professionals
- Providing education and support to patients and their families
Qualifications:- MD/DO degree
- Board certified family practice, internal medicine or emergency medicine physician
- Active and unrestricted medical license in Indiana
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal abilities
- Experience in post-acute care a plus
