Responsibilities: - Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facility
- Provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrollees
- Work alongside pharmacists on medication management
- Complete acute, follow up, advanced care planning, discharge, and wellness visits as necessary
- Implement an individualized plan of care for patients' goals
- Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
- Timely response in chart completions and pharmacy recommendations
Experience as a clinician in a skilled nursing facility preferred. However, physicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), internal (IM), emergency (EM) geriatric, primary, adult, post-acute, home health, hospice, and palliative medicine. Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential.
